Left Atrial Appendage Occlusion Implants (WATCHMAN implants)

Overview: The WATCHMAN procedure is a minimally invasive treatment option designed to reduce the risk of stroke in patients with atrial fibrillation (AFib) who are not suitable candidates for long-term anticoagulant therapy, commonly due to bleeding, falls, or cost issues. This procedure involves implanting a device into the heart’s left atrial appendage (LAA) to prevent blood clots from forming and entering the bloodstream, which can lead to a stroke. This is a therefore a type of left atrial appendage occlusion implant.

What is the WATCHMAN Device?

  • The WATCHMAN device is a small, umbrella-shaped implant made from a combination of nitinol and fabric. It is designed to fit securely within the LAA, a small pouch in the left atrium where blood clots tend to form in patients with AFib.

Procedure Details:

  1. Preparation:
    • Patients undergo a thorough evaluation, including imaging studies like a transesophageal echocardiogram (TEE) and CT scan, to ensure the LAA is suitable for the WATCHMAN device.
    • Patients may need to stop certain medications before the procedure but will need to tolerate a short period of blood thinners.
  2. During the Procedure:
    • Performed under general anesthesia, the procedure typically takes about an hour.
    • A large IV is inserted into the vein in the groin area, through which a catheter is advanced to the heart.
    • Using fluoroscopic and echocardiographic guidance, the WATCHMAN device is delivered to the LAA through the catheter.
    • The device is expanded to fit snugly and securely within the LAA, effectively sealing it off.
    • The catheter is then removed, and the IV is removed.
  3. Recovery:
    • Patients are monitored overnight in the hospital and can go home the next day.
    • Follow-up appointments are scheduled to monitor the device’s position and the heart’s healing process.
    • Patients will need to take blood thinning medication for a short period post-procedure until the device is fully integrated into the heart tissue.

Benefits of the WATCHMAN Procedure:

  • Stroke Risk Reduction: Significantly lowers the risk of stroke in patients with non-valvular AFib.
  • Alternative to Anticoagulants: Provides a viable option for patients who are unable to take blood thinners long-term due to bleeding risks or other issues.
  • Minimally Invasive: Less invasive than open-heart surgery, leading to quicker recovery and fewer complications.
  • Long-Term Solution: Once integrated, the device offers a permanent solution to reducing stroke risk.

Risks and Considerations:

  • Potential Complications: Include bleeding, infection, device-related issues, or embolization. However, these risks are generally low.
  • Follow-Up Care: Essential to ensure the device is functioning properly and sealing the LAA well. Regular check-ups and imaging studies are required.
